Chronic Care Coach Fresher Job

Key responsibilities:

1. Engage with patients through various touchpoint calls to understand their current medical health status, past medical/surgical history, medications, labs, etc.
2. Identify if the patient requires additional services such as medications, labs, dietitian, or physiotherapist
3. Assess patients' social determinants and lifestyle status
4. Effectively gather complete medical history from patients and identify deviations from the recommended care plan
5. Provide psychosocial support to help patients cope with chronic, acute, or terminal illnesses
6. Monitor patient health progress and evaluate outcomes
7. Handle patient queries smartly and respond using pre-defined FAQs
8. Educate patients on chronic conditions, treatment options, and the importance of adhering to care plans
9. Coach patients on compliance with labs, medications, diet, and prescribed treatment plans

Requirements:

1. Demonstrate excellent knowledge and interpretation of medical conditions and terminologies
2. Show effective patient medical history-taking skills
3. Coach patients on compliance with medical parameters
4. Communicate clearly and articulately without ambiguity
5. Exhibit active listening and compassion when engaging with patients
