1)Communicate with patients and caregivers to explain the program's eligibility criteria and benefits. Collect and validate the necessary documents for program enrolment and patient consent. Ensure accurate and organized documentation of patient information and program-related data.
2)Coordinate the delivery of medications to eligible patients, ensuring timely and accurate distribution. Provide monthly refill reminders to patients to ensure the continuity of medication supply. Collaborate with the supply chain team to ensure the efficient and reliable delivery of medications and healthcare products
3)Maintain accurate and up-to-date records in patient CRM systems and Excel spreadsheets. Track patient interactions, program activities, and relevant data to support program evaluation and improvement.
4)Verify the receipt of medications or products by patients or caregivers, addressing any issues or concerns promptly. Provide support to patients by addressing inquiries and guiding them through the program's processes. 5)Continuously improve program effectiveness by providing feedback and suggestions based on patient interactions.
