Lyfe Care Manager Fresher Job (Remote)

Key responsibilities:

1. Work as a dedicated relationship manager of the app
2. Explain app usage and features to the patient and the caregiver
3. Understand the clinical context and ensure a sensitive communication style throughout
4. Monitor adherence to the app via the back-end platform by proactively following up whenever data entry is missed
5. Ensure follow-up appointments with doctors and coaches
6. Update prescriptions on the platform after each consult (offline or online)
7. Continuously update the backend system with changes in the patient s plan for setting reminders, targets, alerts, and goals for different parameters (medications, lifestyle, rehab, etc.) on the basis of inputs from the prescription and coach sessions
8. Work on the management of medicine order placement as and when the refill is required
9. Ensure that the patient is adhering to their prescribed medicine schedule and report any side effects or missed doses
10. Work on diagnostic tests order management; following up after every scheduled diagnostic test to ensure entry of reports on the app
11. Manage close communications with the patient, caregiver, and doctor to ensure offline events (emergency, death, cancellations, etc.)
12. Work on activation of different protocols (emergency, DND, post-discharge, etc.) as required
13. Work on overall subscription and app query management
